  • Care Manager (Ulster County)

    As Care Manager, you'll be responsible for providing the core components of care coordination to individuals with chronic illnesses including HIV. Conduct intakes and comprehensive health assessments to identify needed services. Develop care plans and coordinate with interdisciplinary team of medical providers. Field work involves home visits and accompanying clients to medical appointments when needed. You will also maintain electronic health records/data and provide periodic on-call phone coverage as scheduled.

    MSW or related degree with relevant experience OR LPN with 1 year experience providing case management or medical coordination among multiple providers. Two years relevant experience may be substituted for higher degree. Computer experience must include Word, Excel and use of basic search engines. Access to a car and valid driver's license also required. *Bilingual (English/Spanish) gets salary differential*

  • Project Inform Prevention Specialists (MSM) (Mid-Hudson Valley, 1 position available)


  • As the Project Inform Prevention Specialist you'll deliver STI prevention interventions focusing on the Men who have Sex with Men (MSM) population. Responsibilities include outreach and recruitment activities to engage target populations, the use of traditional, electronic and social media networking strategies, providing HIV/HCV/STI counseling and testing, referring individuals for partner notification, medical, mental health, substance abuse and other appropriate services, providing health education and risk reduction/prevention services in group and one-on-one settings and maintaining appropriate, accurate documentation of all program activities.

    Based out of our Poughkeepsie, NY location, this position provides services in Dutchess, Orange, Sullivan and Ulster counties.

    Bachelor's degree in education, health, human services or related degree with experience in prevention, outreach, counseling in the MSM community. A high school diploma with two years of experience in peer education, HIV/AIDS counseling and/or disease prevention in the MSM community will be considered. Must be able to successfully engage the MSM community in discussing sensitive sexuality issues.
